We have also listed the items that we are required to have from you.  We would be pleased to provide any other information you need or would like to have.

THINGS YOU MAY NEED FROM US:

1-Our Operating Authority

2-Our Surety Bond

3-A copy of our other insurances

4-A copy of our “Quick Pay” policy.

THINGS WE NEED FROM YOU:

1-The Carrier Profile Sheet filled out.

2-A copy of your operating authority.

3-Your insurance, naming VHI Transport as a loss payee.

4-A copy of our motor carrier contract signed.

5-Any special instructions of how to pay you.

6-The W-9 form signed and returned.
